New: indicate yourself what your disability means for your daily life
People with a chronic disease and/or rare condition can use an ICF (International Classification of Functioning, Disability and Health) * to digitally fill in what their disability means for their daily life. For this purpose, Stichting Rare Care World (RCW) and Open Health Hub have developed an online ICF-form that can be saved as a PDF after completion and shared with, for example, doctors, physiotherapists, the municipality, UWV or family. In this way, patients do not have to explain again and again what impact their disability has on their social life.
Form can be linked to other codes and standards
The ICF is an international standard that can be securely exchanged from a PGO (Personal Health Environment) with healthcare providers and institutions. As a result, they can be linked in the future to other codes and standards that record diseases, conditions and results from laboratory research (with LOINC). HL7 provides the standards that enable interoperability of health data.
This gives people the opportunity to manage their own health data.
